Any patient with a medical condition that can be monitored remotely qualifies. Starting 2020, care providers with 300 Medicare patients can potentially add $500,000 a year with CPT 99454, 99457 and 99458 codes alone and practices billing for both CCM and RPM can achieve even higher reimbursements.
